It is imperative that we have your up to date contact details in case we need to contact you, particularly for test results or emergencies.
If you receive a new Medicare Card or Pension / Concession Card please present this to Reception on your next visit. This will ensure that accurate information appears on your documents such as Prescriptions and Referral letters.
If you are booking for a family member, please make the appointment under the appropriate patients name. If the booking is for a New Patient you can save time by having them register electronically.
During the arrival / check in process our team may ask a few questions to confirm your identity but also to confirm that none of your details have changed. This helps us to maintain up to date records also. We thank you for your patience and co-operation.
